Hutchings Consulting and Services will provide comprehensive care management services. Our Medical HIV Care Management services support HIV positive clients to:
- Access medical care
- Ensure medication
- Resolve care issues
- Develop their care plan
- Link with primary medical care
- Obtain medication
- Provide referrals to appropriate support services
- Encourage adherence
Primary activities link a person to primary medical care or services. HIV-positive clients may need secondary activities to meet their medical goals. These secondary activities must have a direct relationship to the client’s HIV clinical outcomes.
The process of care management includes:
- Comprehensive assessment of the client’s needs
- Developing an individualized service plan
- Monitoring clients to assess the plan
- Revising the plan as necessary
