Our Case Management Services Program assists families and individuals to be as self supporting as they are able, to protect children and adults who are in danger of abuse, neglect or exploitation, and to avoid unnecessary institutional care when possible.

The function of case management is to verify the need for additional assistance and to assure that the resident’s needs for personal care, as well as needs for other related health. and social services, are being met.
PATIENT ADVOCACY
We provide Patient Advocacy and Insurance Counseling as a part of modern healthcare. We work with patients to help them navigate the confusing and murky waters of healthcare, from insurance, to costs for procedures and much more.
